How to Interpret Lab Values Fast: The Intern's Morning Labs Method
Every intern I have ever supervised has done the same thing in the first week of July. They open the chart at 6 a.m., see forty numbers, and read them the way you read a cereal box: top to bottom, no priorities, no plan. Then they get to rounds and cannot answer the only question I actually ask, which is "what changed overnight and what are you going to do about it?" This page is the method I wish someone had handed me as an intern. It takes a morning lab panel from a wall of numbers to a sixty second read.
Rule one: the trend beats the value, and the delta beats the absolute
A creatinine of 2.1 means almost nothing by itself. A creatinine of 2.1 that was 2.0 yesterday and 2.2 last week in a dialysis patient is background noise. A creatinine of 2.1 that was 0.9 yesterday is a five alarm fire. Same number, opposite meaning. The single value never tells the story. The direction and the rate of change do.
So before you interpret anything, pull up at least three days of results side by side. Every EMR can do this. If you are reading today's labs in isolation, you are not interpreting labs, you are reciting them. On rounds, present the delta, not the value: "creatinine up from 1.1 to 1.6 overnight" is a sentence that means something. "Creatinine 1.6" is a sentence that makes your senior open the chart to do your job for you.
One more point on reference ranges. The flags in the EMR are set by your lab, and normal ranges genuinely vary between labs and assays. Do not treat the H and L flags as a to do list. A flagged value that is stable for a year is usually that patient's baseline. An unflagged value that has moved fast is often the real finding. The flag column is the least useful column on the page.
The CBC in three questions
Do not read the CBC as eight numbers. Read it as three questions.
1. Is the white count moving, and why?
A rising white count in a hospitalized patient has a short differential: infection, steroids, stress, or a missed source. Look at the differential count before you panic. Steroids classically bump the neutrophil count without many bands. A left shift with bands is a different conversation. And a normal white count does not rule out infection in the elderly or the immunosuppressed, so never let one reassuring number close your mind.
2. Is the hemoglobin stable?
The question is not "is the hemoglobin normal." The question is "is it what it was yesterday." A drop of more than a point or so overnight without an obvious explanation deserves a real look: is there bleeding, was the patient volume resuscitated and diluted, was the draw off a line running fluids? Recheck before you transfuse on a single surprising value, and know your patient's transfusion threshold before the number forces the issue.
3. Are the platelets falling?
A platelet count that is drifting down day after day in the hospital is never boring. Think medications, sepsis, and heparin exposure. A falling platelet count in a patient on heparin products is a specific and time sensitive problem, and it is exactly the kind of thing an intern notices at 6 a.m. and a senior needs to hear about at 6:15.

Watch a good senior resident read a basic metabolic panel. Their eyes go left to right across the top row, then the bottom, and they are asking a fixed question at each stop.
Sodium. Is it moving, and how fast? Slow drifts are usually the patient's disease. Fast moves are usually something we did: fluids, diuretics, free water. Sodium that is correcting faster than intended is a problem you create and a problem you own.
Potassium. This is the number most likely to change your morning. Know the trend, know what replacement was given overnight, and know the magnesium, because low potassium will not stay corrected while the magnesium is low. Have a standard approach to repletion rather than improvising doses at dawn; my approach is on the electrolyte replacement page.
Chloride and bicarbonate. Most interns skip these. Do not. A falling bicarbonate is your earliest cheap clue to an evolving acidosis. When the bicarbonate is low, calculate the anion gap every single time, no exceptions. It takes ten seconds with the anion gap calculator, and a new gap acidosis changes the whole day. If the picture is confusing, get a blood gas and work it through properly with the ABG calculator instead of hand waving.
BUN and creatinine. Read them together and read them against the trend. A creatinine bump with a proportionally larger BUN rise suggests the patient is dry or bleeding into the gut. A creatinine bump after a new medication or a contrast load is a different story. Either way, the response starts with the same three moves: check the urine output, review the med list, and look at what fluids the patient actually received, not what was ordered.
Glucose. Note it, correlate it with the insulin the patient actually got, and move on unless it is extreme.
Calcium. One habit separates interns who understand calcium from interns who chase it: correct for albumin before you react. A "low" total calcium in a malnourished patient with an albumin of 2 is often a normal corrected value. Run it through the corrected calcium calculator before you order a single amp of anything, or check an ionized calcium when it matters.
When the critical value is an artifact
The lab will call you with critical values, and some of them are not real. Two classics every intern must know.
Hemolyzed potassium. A difficult draw, a small needle, a prolonged tourniquet time, or a sample sitting too long can lyse red cells and spill potassium into the tube. The result: a potassium of 6.5 in a patient who feels fine, with a normal creatinine, no potassium sparing meds, and a potassium of 4.0 yesterday. Most labs report a hemolysis index; look for it. The move is a stat redraw, ideally a clean venipuncture without a tourniquet left on forever. But here is the part that matters: if the story is plausible for true hyperkalemia, renal failure, tissue breakdown, an ACE inhibitor plus spironolactone, or the patient has any symptoms, you get an ECG and treat while you wait for the repeat. Calling artifact is a conclusion you are allowed to reach only after the repeat comes back, not before.
Pseudohyponatremia. Severe hypertriglyceridemia or a paraprotein can make the measured sodium read falsely low on some assays, and marked hyperglycemia drags the sodium down by a real but predictable dilutional effect. The concept to carry: a low sodium that does not fit the clinical picture deserves a glucose check, a look at the lipids, and a conversation before anyone starts restricting or infusing. Chasing a fake sodium with real treatment is how you turn a lab curiosity into an actual injury.
The labs that buy a call to your senior
You will not get in trouble for calling your senior about a lab. You will get in trouble for sitting on one. My list of results that justify waking someone up, before rounds, not at rounds:
- A potassium that is critically high or low and confirmed, or unconfirmed with any symptoms or ECG changes.
- A hemoglobin that has dropped substantially overnight without an explanation you can defend out loud.
- A new, significant creatinine rise in a patient who was making urine yesterday.
- A new anion gap acidosis, or a bicarbonate in free fall.
- A rapidly falling platelet count, especially on heparin.
- A sodium moving fast in either direction.
- Any critical value in a patient who also looks worse than yesterday.
Notice the pattern: every item is a delta plus a patient, never a number alone. The overnight version of this judgment, deciding what can wait until morning and what cannot, is its own skill, and I cover it in detail in common night calls.
Treat the patient, not the number
The last trap is the oldest one. Labs are a test of the patient, and the patient is the gold standard, not the printout. If the number says crisis and the patient looks fine, doubt the number first: recheck it, look for artifact, look for a mislabeled tube or a draw above a running IV. If the number says fine and the patient looks sick, doubt the number even harder. Sepsis with a normal lactate on the first draw, a bleed with a not yet equilibrated hemoglobin, and a normal white count in a frail 85 year old with pneumonia are all routine events on a medicine service.
Go see the patient before you treat the panel. It sounds obvious in August. It stops being obvious at 3 a.m. in February, which is exactly when it matters.
